Most Insurance Plans Accepted: 

Dr. Sullivan and Dr. Jackson accept most insurance plans.  They are preferred/ in-network providers for those listed here.  New insurance contract information will be updated when available.  Please note that your insurance website may not reflect these contracts.  You may call the office or check with your insurance plan to verify benefits. 

We routinely check benefits prior to your initial visit.  Starting Jan. 1, 2011 the billing staff will be asking for copays at check-in.  Upon checkout, they may also ask for up to 1/2 of your estimated cost if your deductible has not been met, or for a portion of your projected co-insurance payment if applicable.  Check with your insurance company or call our office for details.

Regarding Estimated Costs:

Allergists are highly specialized and use a number of individual testing reagents to arrive at a diagnosis.  Insurance reimbursement is per reagent so the total cost for a visit may vary.  You may call your insurance company for an estimation ahead of time using the insurance procedure codes below.  You may also be asked for a diagnosis code so you may use 477.9 (allergic rhinitis) to check your benefits, even if this code does not apply.  Your doctor will submit the claim with a code that is appropriate for you at the time of service.

BC/BS average reimbursement for new patient visits including testing: (2 hour average visit)

1. Infant- Age 5 yrs (zero to 20 environmentals + select foods)= $150-500
2. Age 6 yrs - Adult (48 environmentals + select foods + intradermals (x12-17) + spirometry if needed)= $600-1100
